2. Find a report with your name and click the Open Report button next to it (results with Get Full Report buttons should be disregarded). 

3. Copy the URl in the address bar in the next window

4. Navigate back to the home page and at the bottom click on Remove My Info

5. Clicck on Filling Out This Form

6. Fill out all of the requested info in the google form and hit Submit

7. This completes the removal process
