1. Navigate to https://www.mylife.com/ and click on Do Not Sell My Personal Information at the bottom of the page

2. Click on Find URL of Reputation Profile by last name, select the letter of the alphabet that corresponds with your last name

3. Continue selecting names to narrow down th choices until you find your own last name

4. Once you click your name a pop up window with your info will be displayed, close it using the x in the upper right hand

5. Then copy the URL in the address bar

6. Then return to the window with the opt out form. and enter the requested information as well as entering the URL copied in step 5. Also complete the CAPTCHA and hit Opt Out

7. This will remove you from Mylife.com’s database
