Then click Opt-Out Form under how to opt out
Then select next
Then enter your First, middle and last name as well as your email. Complete the CAPTCHA, select the confirmation box and hit Send Request
Navigate to your email inbox and click the blue box to fill out the record removal form
On the next page enter all the required info marked with a red asterisk, select the confirmation box, complete the CAPTCHA and click Submit
