2. Select Opt-Out Form on the next page

3. Select the boxes, provide and email, and complete the CAPTCHA. Then hit Opt-Out My Info

4. Then enter your name, address and state. Press the green magnifying glass

5. Select View All Info next to your profile on the next page.

6. Then press Remove Record

7. Navigate to your email and click the blue box to remove your info
